Notice of Privacy Practices
This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.
Our responsibilities
ClarityPoint Behavioral Health is required by law to maintain the privacy and security of your protected health information (PHI), to provide you with this notice of our legal duties and privacy practices, to follow the terms of the notice currently in effect, and to notify you if a breach occurs that may have compromised the privacy or security of your information.
How we may use and share your health information
- Treatment: to provide and coordinate your care, including with other health care providers involved in your treatment.
- Payment: to bill and receive payment from health plans or other entities.
- Health care operations: to run our practice, improve care, and contact you when necessary.
- As required by law and in other limited situations permitted by law, such as preventing a serious threat to health or safety, public health activities, and responding to lawful requests.
Psychotherapy notes, if any, receive additional protection and generally require your written authorization before they can be shared. Other uses and disclosures not described in this notice will be made only with your written authorization, which you may revoke at any time.

Minors
For patients under 18, parents or legal guardians generally exercise these rights on the minor's behalf, except where Maryland law allows a minor to consent to their own care.
Complaints
You can file a complaint with our office by calling 301-658-2419, or with the U.S. Department of Health and Human Services Office for Civil Rights. We will not retaliate against you for filing a complaint.
